 Students needing Sports Physicals, please do the following:

  • Re-register and/or Register with Arbiter – see Directions below for more information
  • Print out the Physicians Physical Form and present to your Primary Care Doctor
  • Once this form is completed, upload to Students Arbiter Account.
  • Print out and complete the Preparticipation Physical Evaluation Form (two pages)
    • Form should be signed by Parent if you are younger than 18 years old
    • This form should be presented to the Health Center at your appointment.
  • Print out and complete the Consent for Sports Physical Form

 

 

Estimados estudiantes que necesitan un examen físico deportivo, por favor sigan estos pasos:

  • Regístrese o vuelva a registrarse en Arbiter: Consulte las instrucciones para obtener más información.

  • Imprima el Formulario del Examen Físico del Médico y preséntelo a su médico de cabecera.

  • Una vez completado el formulario, súbalo a la cuenta de Arbiter del estudiante.

  • Imprima y complete el Formulario de Evaluación Física Preparticipación (dos páginas).

    • Este formulario debe ser firmado por el padre, madre o tutor si el estudiante es menor de 18 años.

    • Presente este formulario en el Centro de Salud al momento de su cita.

  • Imprima y complete el Formulario de Consentimiento para el Examen Físico Deportivo.
